We work with a wide variety of organizations to provide support for skill development, quality improvement, and system re-design.

The Ministry of Social Development and Poverty Reduction is a provincial government agency that provides British Columbians in need with systems to support them in achieving their social and economic potential. CCMI provided 6 Motivational Interviewing Foundations workshops, with customized materials. As a result of this collaboration, 100 Work BC employment counselors and case managers were trained in the foundational skills and strategies of motivational interviewing,, including opportunities for practice and feedback with qualified MINT members. 91% of the learners rated their intention to use the skills they learned at a 6 or higher (on a scale of 0-7).

The North Central Accountable Community of Health (NCACH) uses collaborative partnerships and innovative solutions to improve the health of their communities as a part of Washington state’s Medicaid Transformation efforts.

The specific aims of the project included:

  • Achievement of the population-based clinical outcome goals of the Medicaid Demonstration project as outlined by the HCA in the Demonstration Project Toolkit, 
  • Build practice capacity for successful adaptation to value-based payment initiatives across payers (e.g., MACRA), including the capacity to deliver effective whole person care and to thrive economically under evolving incentives and reimbursement models.
  • Enable primary care and behavioral health providers in NCW to better integrate behavioral and physical health,
  • Support for addressing social determinants of health at the organizational level,

In partnership with CSI Solutions, CCMI’s involvement in the project included:

  • Collaborating with NCACH in quality improvement and system redesign to refine models of care. 
  • Facilitating virtual engagement and foundation building to ensure success of the collaborative. 
  • Facilitating a  modified BreakThrough Series learning collaborative that combined face-to-face and virtual training, augmented by practice coaching.   
  • Providing virtual skills-based training for self-management support.

Ontario’s self-management programs, operated by the Ministry of Health and Long-Term Care, are responsible for helping people to access home- and community-based care and social services. The purpose of the self-management health care training project is to ensure that healthcare workers within the province have self-management support knowledge and skills in order to support their patients effectively during healthcare interactions. 

  • CCMI provided train-the-trainer workshops and materials for health care providers to become trainers in their own organizations. 
  • Annual trainer review to ensure the quality and fidelity of the training program.

The BC Ministry of Health is responsible for health care services in British Columbia. CCMI partners with the Ministry of Health as part of their Patients as Partners Initiative, which is founded on the principle, “nothing about me, without me.” The work of the initiative is focused on three main areas: person- and family-centred health care, chronic disease self-management, and engagement. 

 

 CCMI’s involvement has included:

  • Creating infrastructure to provide person- and family-centred care. 
  • Skills training in motivational interviewing, Brief Action Planning, health literacy, informed decision-making, and more.
  • Quality improvement projects.
